This is most often caused by a mistake in the original application that is discovered in underwriting. It may the age of the insured or the driving record or any other mistake that would affect the final insurance quote.
It can be caused by either agent or applicant error. To avoid a misquote, provide accurate information on your application. There is no reason to not be accurate as all information provided is verified by the insurer prior to the actual issuance of the policy.
